New Patient Checklist

It is crucial when choosing a pediatric practice for your children that you have given serious consideration to the fit between the practice philosophies and your own.


Excellent care happens when the physicians and office staff are aligned with your family priorities and philosophies regarding medical care. Only then can shared decision-making be based on mutual respect.


Please read the following checklist carefully to be sure we are  the appropriate medical home for your children.

  • Office Hours

    We make every effort to meet the needs of our patients, offering both routine well visits and sick visit appointments.


    If your child needs an appointment self-schedule an appointent at any time or call our office between 8am-5pm. 


    ​Please familiarize yourself with our locations to see if they work for your family.


    Outside of normal business hours, please reference our website for resources, including self-scheduling appointments, electronic bill pay, medication dosages, and is my child sick.  Please refer to our after hours for additional help when we are closed.

  • Vaccines

    We firmly believe in the safety and effectiveness of vaccines to prevent serious illness and to save lives.  We are more than willing to discuss any questions you may have about vaccines, but do require all patients to our practice to adhere to the vaccination schedule outlined by the Centers for Disease Control and Prevention (CDC) and the American Academy of Pediatrics (AAP).  These routine vaccines are given at well child visits. 


    Additionally, seasonal vaccines (like the flu shot) can also be given at sick visits or vaccine only visits (click here to schedule a vaccine only visit).  If your child is nervous about vaccines, check out our vaccine bravery plan below.

  • Technology

    Our practice prides itself on efficiency through use of technology. We encourage you to consult our website for resources and use our patient portal, where you can message our team, place refill requests, or print shot records.  At any time of day, you can convieniently electronically complete visit preregistration and surveys, pay co-pays and bills, and self-schedule appointments (click here to schedule a visit).  Telehealth appointments are available via Zoom for certain visit types, like behavioral health.

  • After Hours

    We have a physician on call 24/7/365. When our office is closed overnight and you have an urgent issue that cannot wait until morning, you can call our main number for advice. 



    Please familiarize yourself with our Office Hours.


    Self-scheduling Visits is available at any time of day.


    Please call during office hours to schedule, reschedule, or cancel an appointment.


    Medication refills can be requested through the Patient Portal or during office hours.

  • Insurance

    Make sure we participate with your insurance plan. 


    It is your responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays at the time of service.


    Our billing staff will do their best to assist you with insurance questions; however, If you have questions about your coverage, it is best check with your specific insurance company.


    Our office does not want you to be surprised by a bill, but must always bill your health plan based on federal guidelines and the actual services provided.

  • Antibiotics

    We work hard to not overuse antibiotics. We educate families on evidence-based guidelines and don’t automatically treat symptoms with antibiotics.  Medications can have side effects and can be hard to administer to children, so we only prescribe them when necessary.


    We do not routinely prescribe antibiotics over the phone as we do not believe that is good medicine.  Please call our office or self-schedule an illness visit to make an appointment to be seen. 

  • Medical Home

    We work hard to provide comprehensive medical care and serve as your medical home.  Let us be your first stop with any concern about your child.  We want to be involved in either providing care in our office where appropriate, or referring you to the most appropriate specialist and helping to coordinate your care.  Seeing us at a scheduled time in

    the office often means less time in a waiting room and a lower bill (compared to the Urgent Care or Emergency Department).
